Process for Developing and Revising SCAO-Approved
Court Forms
The Trial Court Services Division of SCAO is responsible for developing,
revising, approving, and distributing court forms. Part of that process
includes review and recommendation by the Michigan Court Forms Committee.
Comments and requests for new or revised forms will be published on this
website for a 30-day comment period. The comments received during the
publication period will be provided to the Michigan Court Forms Committee
for discussion at its meetings according to the following schedule:
The Trial Court Services Division of SCAO is responsible for developing, revising, approving, and distributing court forms. Part of that process includes review and recommendation by the Michigan Court Forms Committee. Comments and requests for new or revised forms will be published on this website for a 30-day comment period. Comments received during the publication period will be provided to the Michigan Court Forms Committee for discussion at its work group meetings according to the following schedule:
- Adoption Work Group: Comments and requests received through December 31, 2011, will be published by January 11, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on February 23, 2012. Comments and requests received after December 31, 2011, will not be published until January of 2013. ** Note: This work group will not meet in 2012.
- Criminal Work Group: Comments and requests received through December 31, 2011, will be published by January 18, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on March 1, 2012. Comments and requests received after December 31, 2011, will not be published until January of 2013.
- General Civil and Miscellaneous Work Group: Comments and requests received through December 31, 2011, will be published by January 25, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on March 8, 2012. Comments and requests received after December 31, 2011, will not be published until January of 2013.
- Domestic Relations Work Group (including Friend of the Court): Comments and requests received through December 31, 2011, will be published by February 1, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on March 15, 2012. Comments and requests received after December 31, 2011, will not be published until February of 2013.
- Civil Infractions, Other Civil, and Summary Proceedings Work Group: Comments and requests received through December 31, 2011, will be published by February 8, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on March 22, 2012. Comments and requests received after December 31, 2011, will not be published until February of 2013. ** Note: This work group will not meet in 2012.
- Estates and Trusts Work Group: Comments and requests received through June 30, 2012, will be published by July 18, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on September 5, 2012. Comments and requests received after June 30, 2012, will not be published until July of 2013. ** Please note date change.
- Mental Health/Commitment Work Group: Comments and requests received through June 30, 2012, will be published by July 25, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on September 13, 2012. Comments and requests received after June 30, 2012, will not be published until July of 2013.
- Child Protective Proceedings and Juvenile Guardianship Work Group: Comments and requests received through June 30, 2012, will be published by August 1, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on September 20, 2012. Comments and requests received after June 30, 2012, will not be published until August of 2013.
- Guardianship, Conservatorship, and Protective Proceedings Work Group: Comments and requests received through June 30, 2012, will be published by August 8, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on September 27, 2012. Comments and requests received after June 30, 2012, will not be published until August of 2013.
- Delinquency, Designated, Minor Personal Protection, and Traffic/Ordinance Work Group: Comments and requests received through June 30, 2012, will be published by August 15, 2012, for a 30-day comment period and then referred to the work group for discussion at a meeting on October 4, 2012. Comments and requests received after June 30, 2012, will not be published until August of 2013.
The Michigan Court Forms Committee consists of representatives from various trial court associations, some sections of the State Bar of Michigan, and relevant state departments or agencies. The Michigan Court Forms Committee is comprised of ten work groups that meet annually and discuss requests for new forms and suggestions for revising existing forms that are received throughout the year by the Trial Court Services Division of SCAO. Forms approved by the committee are recommended to the State Court Administrator for approval. Forms approved by the State Court Administrator are then distributed to trial courts, printers, publishers, and relevant state departments semiannually and posted to the SCAO website. Uniform Law Citation
- Courts that want to revise the Uniform Law Citation for local purposes
must submit a draft to the Trial Court Services Division of SCAO for review.
If the revision is approved, the draft will be forwarded to the Attorney
General, Secretary of State, and Director of Michigan State Police for
approval as required by Michigan law. See

Changes to Landlord-Tenant and Small Claims Forms. Forms used in landlord-tenant and small claims proceedings have been revised by a work group of the Solutions on Self-Help (SOS) Task Force. These forms are available in Zip files. To unzip each

from the files. Note: The landlord-tenant forms were revised for the benefit of pro se litigants and primarily for completion online. Courts are not required to provide these forms and/or the instructions and should make their own policies and decisions in this regard.

Forms with Specific Effective Dates
Effective January 1, 2012
PCM 201, PCM 214, PCM 242 and PCM 243 were revised to comply with a new mandate that all mental health orders entered into the Law Enforcement Information Network (LEIN) must contain the place of birth. The revised forms are dated 11/11. The current versions of these forms cannot be used after December 31, 2011. See the index.
Effective January 1, 2012
The Uniform Support Orders (FOC 10/52, 10a/52a, 10b, 10c) were revised and are dated 3/11. These forms can be used immediately. Use of these forms is mandatory as of January 1, 2012. See the index.
Effective May 1, 2010
Adoption, guardianship, and juvenile forms are available for use with
proceedings involving Indian children pursuant to amended court rules, effective May 1, 2010. See above for details.

Use of Previous Versions of SCAO-Approved Forms
When SCAO-approved forms are revised, instructions are provided in the numerical indexes about using existing stock of previous versions of those forms. "Existing stock" means preprinted NCR or carbon-interleaved paper stock of the forms only. It does not include print-on-demand paper forms that are generated electronically from a computer. Previously-approved versions of any revised SCAO-approved forms that are reproduced in electronic format must be updated as soon as the new SCAO-approved versions are published irrespective of instructions regarding use of existing stock.

Numerical and Alphabetical Indexes of Forms
These indexes list each form number, the most recent revision date
of that form, the name of the form, associated citations, and previous
revision dates and information about continued use of old versions of
printed forms. Forms cannot be opened through
the indexes; please see the "Categories
of Forms" below.
When a form is revised, a note is added to the numerical index indicating
whether stock of previous versions of that form can be used until it
is depleted.

Categories of Court Forms
Selecting a Form
The forms are grouped by category. If you know the category of a form,
select the category first. Then select the specific form you want from
the alphabetical list in that category.
If you don't know what category a form is in, use the "Search"
window at the top of this page using either the name or number of the
form. If you know the form number, the search will return the direct
link to the form only if you enter the identifier and number without
spaces. For example, to locate the Subpoena (form MC 11), enter "mc11.pdf"
or "Subpoena."
NOTE: To view the forms, you must have Adobe Acrobat Reader 6.0
or higher installed on your computer. To download your free copy from
Adobe's web site, click "here."
Some web users have reported problems viewing PDF files through their
respective web browsers. To circumvent this problem, please right click
on the link and save the file to your computer.
Completing Forms Online
The PDF forms on this site can be filled in online and printed or
they can be downloaded - this ability comes from free software provided
by Adobe Acrobat called Acrobat Reader. Forms that have been
filled in online cannot be saved without the full version of Adobe
Acrobat. For information about the cost of this software and how to
purchase it, see Adobe's
web site.
Printing Completed Forms for Filing with the Court
You are required to provide the total number
of copies of a document or form for use by the court, the parties, and
any other entity that is required to receive the form. See
the upper-right hand corner of each form to determine the mandatory
number of copies. It is recommended that when completing a form online,
you print the number of copies you will need for filing with the court
and serving on the parties. Otherwise, you will have to photocopy the
proper number of copies before filing the form with the court. If you
do not provide the court with the accurate number of copies, the court
might reject the form for nonconformance under the authority of Michigan
Court Rule 8.119(C). Unless specifically required by court rule or statute,
the court is not responsible for making copies of forms for you.
