Forms for Use in
Recording and Reporting Disposition of Criminal Cases
This set of forms is
for use in recording the disposition of criminal cases. See also the general
criminal index for other dispositional forms. Some of these forms must
be transmitted to the Michigan State Police, Criminal Justice Information Center,
upon completion. MC forms are used in either circuit or district court.
CC forms are used only in circuit court. DC forms are used only in district
court. See also the general, general criminal, felony criminal, misdemeanor
criminal, motor vehicle offense, and probation indexes for forms which
are used in criminal matters as appropriate.
