Process for Developing and Revising SCAO-Approved Court Forms

The Trial Court Services Division of SCAO is responsible for developing, revising, approving, and distributing court forms. Part of that process includes review and recommendation by the Michigan Court Forms Committee. Comments and requests for new or revised forms will be published on this website for a 30-day comment period. The comments received during the publication period will be provided to the Michigan Court Forms Committee for discussion at its meetings according to the following schedule:

  • Circuit Court (CC) forms: Comments and requests received through December 31, 2008, will be published by January 19, 2009, for a 30-day comment period and then referred to the committee for discussion at a meeting on March 5, 2009. Comments and requests received after December 31, 2008, will not be published until January of 2010.


  • District Court (DC) forms: Comments and requests received through December 31, 2008, will be published by January 19, 2009, for a 30-day comment period and then referred to the committee for discussion at a meeting on March 5, 2009. Comments and requests received after December 31, 2008, will not be published until January of 2010.


  • Family Division of Circuit Court (JC, PCA, and CCFD) forms: Comments and requests received through June 30, 2008, will be published by July 28, 2008, for a 30-day comment period and then referred to the committee for discussion at a meeting on September 18, 2008. Comments and requests received after June 30, 2008, will not be published until July of 2009.


  • Friend of the Court (FOC) forms: Comments and requests received through December 31, 2008, will be published by February 2, 2009, for a 30-day comment period and then referred to the committee for discussion at a meeting on March 19, 2009. Comments and requests received after December 31, 2008, will not be published until January of 2010.


  • Michigan Court (MC) forms: Comments and requests received through December 31, 2008, will be published by January 19, 2009, for a 30-day comment period and then referred to the committee for discussion at a meeting on March 5, 2009. Comments and requests received after December 31, 2008, will not be published until January of 2010.


  • Probate Court (PC and PCM) forms: Comments and requests received through June 30, 2008, will be published by July 21, 2008, for a 30-day comment period and then referred to the committee for discussion at a meeting on September 4, 2008. Comments and requests received after June 30, 2008, will not be published until July of 2009.


The Michigan Court Forms Committee consists of representatives from various trial court associations, some sections of the State Bar of Michigan, and relevant state departments or agencies. Individual sections of the Michigan Court Forms Committee meet annually and discuss requests for new forms and suggestions for revising existing forms that are received throughout the year by the Trial Court Services Division of SCAO. Forms approved by the committee are recommended to the State Court Administrator for approval. Forms approved by the State Court Administrator are then distributed to trial courts, printers, publishers, and relevant state departments semiannually and posted to the SCAO website. Uniform Law Citation - Courts that want to revise the Uniform Law Citation for local purposes must submit a draft to the Trial Court Services Division of SCAO for review. If the revision is approved, the draft will be forwarded to the Attorney General, Secretary of State, and Director of Michigan State Police for approval as required by Michigan law. Forms with Specific Effective Dates

Effective Immediately, September 24, 2008
The Request and Writ for Garnishment (Income Tax Refund/Credit) has been revised because the address for the Department of Treasury's Third Party Withholding Unit has been changed, effective immediately. Anyone using preprinted stock of the form may continue to use the stock until it is depleted; however, after July 1, 2009, the address must first be corrected before continuing to use preprinted stock.
Effective November 1, 2008
The distribution of the first copy of MC 288, Order to Remit Funds for Fines, Costs, and Assessments, and MC 290, Satisfaction of Financial Obligation, was changed from the institution or facility to the Michigan Department of Corrections, Regional Business Office. Completed forms should be sent to the following address, effective November 1, 2008: Michigan Department of Corrections, Region III Business Office, Court Order Processing, 4000 Cooper Street, Jackson, MI 49201.
Effective October 1, 2008
The Uniform Support Orders for child support were revised to comply with amendments to the Child Support Formula, effective October 1, 2008, and the Uniform Support Orders for spousal support were revised in response to a request, also effective October 1, 2008. See the index.
Effective September 1, 2008
The civil default and judgment forms (MC 07, MC 07a, and MC 10) were revised to comply with an amendment to MCR 2.603, effective September 1, 2008. All three forms contain a revision date of 8/08 and should be used starting September 1, 2008. See additional information about use of previous versions. >>

Use of Previous Versions of SCAO-Approved Forms

When SCAO-approved forms are revised, instructions are provided in the numerical indexes about using existing stock of previous versions of those forms. "Existing stock" means preprinted NCR or carbon-interleaved paper stock of the forms only. It does not include print-on-demand paper forms that are generated electronically from a computer. Previously-approved versions of any revised SCAO-approved forms that are reproduced in electronic format must be updated as soon as the new SCAO-approved versions are published irrespective of instructions regarding use of existing stock.

These indexes list each form number, the most recent revision date of that form, the name of the form, associated citations, and previous revision dates and information about continued use of old versions of printed forms. Forms cannot be opened through the indexes; please see the "Categories of Forms" below. >>

When a form is revised, a note is added to the numerical index indicating whether stock of previous versions of that form can be used until it is depleted.

    Categories of Court Forms

    Selecting a Form

    The forms are grouped by category. If you know the category of a form, select the category first. Then select the specific form you want from the alphabetical list in that category.

    If you don't know what category a form is in, use the "Search" window at the top of this page using either the name or number of the form. If you know the form number, the search will return the direct link to the form only if you enter the identifier and number without spaces. For example, to locate the Subpoena (form MC 11), enter "mc11.pdf" or "Subpoena."

    NOTE: To view the forms, you must have Adobe Acrobat Reader 5.0 or higher installed on your computer. To download your free copy from Adobe's web site, click  "here". Some web users have reported problems viewing PDF files through their respective web browsers. To circumvent this problem, please right click on the link and save the file to your computer.

    Completing Forms On-Line

    The PDF forms on this site can be filled in on-line and printed or they can be downloaded - this ability comes from free software provided by Adobe Acrobat called Acrobat Reader. Forms that have been filled in on-line cannot be saved without the full version of Adobe Acrobat. For information about the cost of this software and how to purchase it, see Adobe's web site.

    Printing Completed Forms for Filing with the Court

    You are required to provide the total number of copies of a document or form for use by the court, the parties, and any other entity that is required to receive the form. See the upper-right hand corner of each form to determine the mandatory number of copies. It is recommended that when completing a form on-line, you print the number of copies you will need for filing with the court and serving on the parties. Otherwise, you will have to photocopy the proper number of copies before filing the form with the court. If you do not provide the court with the accurate number of copies, the court might reject the form for nonconformance under the authority of Michigan Court Rule 8.119(C). Unless specifically required by court rule or statute, the court is not responsible for making copies of forms for you.
